Health records are protected health information that cannot be released to any person or organization without the consent of the patient or the patient’s legally authorized representative (unless authorized or required by law).
Access Your Records Online (MyChart)
UT Austin School of Nursing provides access to patients for portions of their health record—including test and lab results, immunization history, after visit summaries and notes, provider messaging and more—through a patient web portal called MyChart.

Records may also be requested by filling out an authorization form, electronically submitting an authorization form online, or mailing/faxing an authorization form.
